Privacy Policy

1. Information We Collect
We collect the following categories of information:
- Information you provide. Name, phone number, email address, and any details you share in a form or phone call, including whether fusion has been recommended and a brief description of your symptoms.
- Automatically collected data. IP address, device and browser type, operating system, referring URL, pages viewed, time on page, clicks, scroll depth, and approximate location derived from IP.
- Advertising and campaign data. Google Click ID (GCLID), Google Ads campaign, ad group, keyword, and UTM parameters that identify how you arrived at the site.
- Call tracking data. When you call a number displayed on this site, our call tracking provider (CallRail) may dynamically swap the number, record the call, capture the calling number, call duration, and the marketing source that drove the call.
2. How We Use Your Information

- Return phone calls, texts, and emails you initiate.
- Measure the performance of our website, advertising, and content.

3. SMS/Text Messaging Consent
When you submit a form on this site, you consent to receive calls, text messages, and emails from Ocean Spine & Pain (Justin Kubeck, MD) at the phone number and email address you provide. Messages may include appointment scheduling, reminders, follow-up care, and other practice-related communications. Message and data rates may apply.
You may opt out of text messages at any time by replying STOP or following the unsubscribe instructions included in any message. You may also contact our office at the phone number below to update your communication preferences.
4. Advertising, Analytics, and Tracking Technologies
This site uses cookies, pixels, SDKs, server-side tagging, and similar technologies from the following providers. These tools help us understand which ads and pages drive consultation requests so we can spend advertising dollars responsibly.
- Google Analytics 4 (GA4). Measures pageviews, events, and conversions. IP addresses are truncated by Google before storage.
- Google Ads. Conversion tracking, remarketing, and enhanced conversions. Hashed (SHA-256) versions of email and phone number may be sent to Google to match conversions to ad clicks. We do not send raw personal information to Google.
- Google Tag Manager. Manages the tags above; it does not itself set marketing cookies.
- Meta (Facebook/Instagram) Pixel and Conversions API. If enabled, measures conversions and supports remarketing on Meta platforms using hashed identifiers.
- Microsoft Advertising (Bing UET). If enabled, measures conversions from Microsoft ads.
- CallRail. Dynamic Number Insertion (DNI), call recording, and call attribution.
- Session and performance tools. We may use lightweight tools to record aggregated interaction data (clicks, scrolls, form errors) to improve the site.
This site is intended for a New Jersey audience. We do not currently display a cookie consent banner. If you visit from a jurisdiction that requires prior consent for advertising or analytics cookies, please use your browser controls or the opt-out links in Section 5 to limit tracking.
5. Your Choices and Opt-Outs
- Adjust your browser settings to block or delete cookies.
- Opt out of Google Ads personalization: adssettings.google.com.
- Opt out of GA4 via the Google Analytics Opt-out Browser Add-on.
- Opt out of interest-based advertising across participating networks: optout.aboutads.info and optout.networkadvertising.org.
- Opt out of SMS/text messages at any time by replying STOP to any message or contacting us at the phone number below.
6. How We Share Information
We do not sell your personal information for money. We share information only with:
- Service providers who host the site, deliver forms, provide analytics, run call tracking, or manage advertising, under contracts that limit their use of the data.
- Advertising platforms as described above, generally using hashed identifiers.
- Legal or regulatory authorities when required by law, subpoena, or to protect rights and safety.
Under some U.S. state privacy laws (including CCPA/CPRA), the transfer of online identifiers to advertising platforms for cross-context behavioral advertising may be considered "sharing." You may opt out using the mechanisms in Section 5.
7. Data Retention
We retain form submissions and call records for as long as necessary to respond to your request, provide care coordination, and satisfy legal, accounting, or reporting requirements. Analytics and advertising data are retained according to each provider's default retention (typically 14 months for GA4 event data).
8. Security
We use industry-standard technical and organizational measures, including TLS encryption in transit and access controls, to protect information collected through this site. No method of transmission over the internet is 100% secure.
9. Your Privacy Rights
Depending on where you live, you may have the right to access, correct, delete, or receive a copy of your personal information, to opt out of sale/sharing or targeted advertising, and to appeal a denial of your request. To exercise any of these rights, contact us at the phone number below. We will verify your request using information reasonably necessary to confirm your identity.
10. HIPAA and Health Information
Information submitted through this website's forms or chat is used solely to respond to your inquiry and is not maintained as a HIPAA-covered medical record. Once you become a patient, protected health information is handled under our Notice of Privacy Practices provided at the practice.
11. Children's Privacy
This site is not directed to children under 13, and we do not knowingly collect personal information from children under 13.
12. Third-Party Links
This site may link to third-party sites. We are not responsible for the privacy practices or content of those sites.
13. Changes to This Policy
We may update this Privacy Policy from time to time. Updates take effect when posted on this page with a new effective date.
